TEMPORARY ACCESS CARD FORM

**This form is used for a property owner visiting Sea Trail and has forgotten their access card at home**

DURATION OF A TEMPORARY CARD WILL NOT EXCEED 30 DAYS

(An access card is needed to enter pools at the MAC and Pink Palace, Tennis Courts, Library, Chapel, Fitness Center, Spa and Beach Parking Lot.)

TEMPORARY ACCESS CARD FORM

After this form is submitted an access committee member or the STMA office will meet with the HOA member and issue a card. A $25.00 fee will be charged to the HOA members account. The card cannot be used for more than 30 days. If the card is lost or returned damaged there will be a $50.00 fee charged to the HOA Members account .The card must be picked up in person.
