PROPERTY OWNER’S LOST ACCESS CARD REPLACEMENT FORM

**This form is used when a property owner requests a new card to replace their lost card**

(An access card is needed to enter pools at the MAC and Pink Palace, Tennis Courts, Library, Chapel, Fitness Center, Spa and Beach Parking Lot.)

ACCESS CARDS WILL BE REPLACED AT A COST OF $25.00 EACH, PAYABLE BY CHECK TO THE SEA TRAIL MASTER ASSOCIATION UPON PICKUP
Property Owner’s Access Cards shall be issued only to those entitled under the STMA RULES and subject to the following conditions:
A maximum of TWO Property Owner’s Access Cards may be issued associated with a single property in good standing, regardless of the number of property owners as defined in the STMA RULES. No property owner, as defined in the STMA RULES, shall be entitled to issuance of a card related to more than one property, regardless of how many properties for which a property owner may otherwise qualify in Sea Trail.
By accepting issuance of this Property Owner’s Access Card, and my signature below, I acknowledge and represent that I am familiar with the STMA RULES as applicable to the use of all STMA common facilities and my obligation to comply with those rules in the use of this Card. I acknowledge that I will be liable and responsible for compliance with the STMA RULES and for any damages which may result from the use of this Card. I further acknowledge and understand that it is my responsibility to notify STMA if my card is lost or stolen, damaged or if I am no longer qualified to possess this access card. I further acknowledge that I will be charged a fee of $25 for replacement of the card and that use of this card in violation of the STMA RULES may result in fines, suspension of privileges to common and limited common facilities and collection action pursuant to adopted STMA collection policy and North Carolina Statutes.

WAIVER OF LIABILITY

By my signature requesting issuance of this card, I acknowledge that I am utilizing the STMA common facilities at my own risk and peril, waive any liability of STMA for any damages which may reasonably result from my use of this card, and assume the responsibility for compliance with the STMA RULES for me, my family members and for the actions of my guests and anyone to whom I may give permission to use this card while using the common and limited common facilities. Requester MUST sign before submitting this form.
