(An access card is needed to enter pools at the MAC and Pink Palace, Tennis Courts, Library, Chapel, Fitness Center, Spa and Beach Parking Lot.)

3. WAIVER OF LIABILITY, ACKNOWLEDGEMENT AND AUTHORIZATION

By granting permission for the use of my Property Owner’s Access Card and my signature below, I acknowledge and represent that I am familiar with the Sea Trail Master Association RULES as applicable to the use of all common facilities and that I have made known to the individual(s) I am authorizing of their obligations to comply with those rules and regulations in the use of my Property Owner Access Card. I further acknowledge and represent that I am aware that notwithstanding the granting of my permission in this Authorization, I continue to remain fully liable and responsible for compliance with the STMA RULES, the conduct of those individuals to whom I am authorizing use, and for any damages which may result from my granting of permission to the individuals designated above. I further acknowledge and understand that my privileges under my Property Owner’s Access Card may be suspended, de-activated, or forfeited as a result of any violation of the STMA RULES resulting from the conduct of the individuals for whom I am authorizing use, and that I may be subject to monetary penalties for violations, and re-activation fees. Property owner MUST sign before submitting this form. *
